Dear Ms. Sharp:
I certainly was saddened that Ms. Deltuva did so badly with this
cancer. It is also unfortunate that the quickness of her death
prevented us from diagnosing it anymore specifically than was done.
Basically nothing more is known than what you have already been told.
She presented on 5~6-85 with a partial bowel obstruction at the office
and she died on 6-17-85. The cancer was spread through out the
abdominal cavity and it was not possible to determine where it
initially started. The only clue in the pathology report was that it
was an adeno carcinoma. This suggest*that it may have started in some
sort of glandular structure, such as a bowel wall or perhaps the
pancreas. It was not from her breast. The tissue had degenerated
several weeks later when it was seen at IU and their suggestion that
it may represent a lymphoma was based on uncertainty because of some
of the degeneration. There was no question in the minds of our
pathologist when they saw the tissue fresh that it was not a
lymphomatous origin and that it was an adeno carcinoma. Since it was
spread all over the entire abdomen when it was first discovered, there
was no treatment that could really be offered. Chemotherapy might have
prolonged her life slightly but I don't believe that it would have
done anything more than make her miserable for a few extra weeks. This
sort of cancer is extremely aggressive and is not always easily
controlled. Radiation, which I recommend was probably not a good idea
either. As far as the risk to the rest of the family, you should
merely have regular examinations and check your stool for blood. High
fiber diets protect to some degree against malignancies of the bowel
and certainly the women should have regular evaluations of their
breasts. Once or twice yearly examinations of the stool for blood is a
good screening test for adeno carcinoma of the bowel. Certainly anyone
who is concerned about the possibility of cancer should avoid smoking,
but beyond that there really are not a lot of things that should be
done and I believe that you are unlikely to get this kind of cancer
just because your mother got it.
She was a fine lady and I certainly was sorry to see her have this
kind of end. We did everything we could to keep her comfortable and I
do believe that we prevented her from suffering as much as possible.
Sincerely yours,
Edward S Warren, M.D.
